ABSTRACT

Fulvic acid, which has antioxidant, anti-inflammatory, and immunomodulatory properties, is of particular interest in the treatment of oral lichen planus. Objective: to evaluate the regenerative potential of fulvic acid in the combination treatment of erosive and ulcerative oral lichen planus. Materials and methods. We observed 49 patients (35 women, 14 men), average age 48.5 ±5.8 years, diagnosed with oral lichen planus, characterized by atrophic and erosive manifestations. All patients were prescribed combination treatment. For oral hygiene, they were divided into two groups based on the hygiene products used. The main group (n = 25) included the use of fulvic acid toothpaste, while the comparison group (n = 24) used toothpaste containing sodium fluoride, xylitol, and sage and chamomile extracts. Regeneration dynamics were assessed at follow-up points – days 14, 28, 42, and 49 – by evaluating the area of erosive defects. Statistical processing was performed using Statistica and Excel. Results. In subgroup A with an erosion area of less than 20 mm² in the main group, complete epithelialization of erosive defects was achieved in 100 % of patients by day 28, compared to 92 % in the comparison group (p < 0,05). In subgroup B with an erosion area of 20 mm² or more in the main group, complete epithelialization (100 %) was achieved by day 49, compared to 92 % in the comparison group (p < 0,05). Conclusions. The study confirmed the regenerative and anti-inflammatory potential of fulvic acid in the complex treatment of erosive and ulcerative lichen planus of the oral mucosa.
