Predmet. Poisk ob'ektivnyh skrinigovyh metodov diagnostiki oslozhneniy dental'noy implantacii, obladayuschih vysokoy chuvstvitel'nost'yu i specifichnost'yu, a takzhe sluzhaschih indikatorom doklinicheskoy stadii razvitiya periimplantita i prognozirovaniya effektivnosti osteointegracii. Cel' issledovaniya. Izuchenie dinamiki izmeneniy urovnya okislitel'noy modifikacii belkov v slyune dlya razrabotki metoda ranney diagnostiki oslozhneniy pri dental'noy implantacii. Metodologiya. V issledovanii prinyali uchastie 84 pacienta, kotorym byla proizvedena operaciya dental'noy implantacii s primeneniem razlichnyh implantacionnyh sistem. Osnovnuyu gruppu sostavili 53 pacienta s diagnostirovannymi oslozhneniyami, razvivshimisya posle implantacii; gruppa sravneniya sformirovana iz 31 pacienta s blagopoluchnym ishodom lecheniya; v gruppu kontrolya voshli 13 prakticheski zdorovyh lic, kotorym operaciya dental'noy implantacii ne provodilas'. Diagnostika periimplantita osuschestvlyalas' v hode klinicheskogo, rentgenologicheskogo i laboratornogo issledovaniy pri povtornom obsledovanii pacientov. V slyune, poluchennoy na etape podgotovki k dental'noy implantacii, opredelyali uroven' okislitel'noy modifikacii belkov. Povtornyy zabor slyuny osuschestvlyalsya cherez chetyre mesyaca posle dental'noy implantacii (pri ee blagopoluchnom ishode) ili v sluchae verifikacii periimplantita. Rezul'taty. Ustanovleno, chto razvitie oslozhneniy dental'noy implantacii soprovozhdaetsya okislitel'nym stressom i nakopleniem produktov okislitel'noy modifikacii belkov v slyune. Povyshenie salivarnogo urovnya okislitel'noy modifikacii belkov pri dental'noy implantacii nablyudaetsya zadolgo do formirovaniya klinicheski znachimyh izmeneniy v tkanyah implantacionnogo kanala i sopryazheno s vysokim riskom razvitiya vospalitel'nyh oslozhneniy dental'noy implantacii. Vyvody. Vysokiy ishodnyy uroven' okislitel'noy modifikacii belkov slyuny mozhet yavlyat'sya nadezhnym prognosticheskim priznakom periimplantita.
