Предмет исследования — распределение окклюзионных сил по зубным рядам (окклюзионный баланс зубных рядов). Цель — изучить характер изменений распределения окклюзионных нагрузок по функциональным отделам зубных дуг у пациентов после тотальной реконструкции зубных рядов с использованием разработанного способа оценки окклюзионного баланса зубных рядов. Методология. Исследование проведено на кафедре ортопедической стоматологии Волгоградского государственного медицинского университета (клиническая база — ГАУЗ «Клиническая стоматологическая поликлиника № 3», г. Волгоград). В исследовании приняли участие 15 мужчин и 17 женщин, нуждающихся в тотальной реконструкции зубных рядов. Средний возраст участников клинического исследования составил 33,4 ± 5,3 года. Окклюзионный баланс оценивали на основе данных электронной окклюзиографии по авторской методике. Этапы наблюдений: перед лечением; через 2 недели после фиксации временных зубных протезов; на следующий день после фиксации постоянных протезов; через 6 месяцев после фиксации постоянных протезов. Результаты. Перед лечением показатели окклюзионного баланса зубных рядов у пациентов варьировали в значительной степени в зависимости от наличия дефектов зубных рядов, их локализации и протяженности. Фиксация временных, а затем постоянных зубных протезов сопровождается выравниванием окклюзионного баланса. Через 6 месяцев после тотальной реконструкции зубных рядов наблюдалось выраженное увеличение коэффициентов распределения жевательной нагрузки во всех функциональных отделах зубных рядов, преимущественно справа. При этом состояние силового баланса окклюзии зубов сохраняется для зубного ряда в целом и ее переднего, левого бокового отделов, а в боковом отделе справа развивались достоверные признаки дисбаланса окклюзии. Выводы. Изменение окклюзионного баланса после тотальной реконструкции зубных рядов, являющееся следствием изменяющегося тонуса мышц стоматогнатической и постуральной систем в процессе их адаптации к новому положению нижней челюсти, необходимо учитывать при организации наблюдения за пациентами после ортопедического стоматологического лечения с целью профилактики постпротетических осложнений.

Subject. The distribution of occlusal forces in the dentition (occlusal balance of the dentition). Objectives. To study the nature of changes in the distribution of occlusal forces in the sections of the dental arches in patients after a total reconstruction of the dentition using the developed method for assessing the occlusal balance of the dentition. Methodology. The study was carried out at the Department of Orthopedic Dentistry with a course of clinical dentistry at the Volgograd State Medical University (clinical base – Dental Polyclinic No. 3, Volgograd). The study involved 15 men and 17 women in need of a total reconstruction of the dentition. The mean age of participants in the clinical study was 33.4 ± 5.3 years. The occlusal balance was assessed on the basis of electronic occlusiography data according to the author's method. Stages of observation: before treatment; 2 weeks after fixing temporary dentures; the next day after the fixation of permanent prostheses; 6 months after prosthetics. Results. Before treatment, the indicators of the occlusal balance of the dentition in patients varied to a large extent depending on the presence of defects in the dentition, their location and extent. Fixation of temporary and then permanent dentures is accompanied by alignment of the occlusal balance of the dentition. 6 months after the total reconstruction of the dentition, there is a pronounced increase in occlusal imbalance in all parts of the dental arches, mainly on the right. Conclusions. The change in the occlusal balance after the total reconstruction of the dentition, which is a consequence of the changing tone of the muscles of the stomatognathic and postural systems in the process of their adaptation to the new position of the lower jaw, must be taken into account when organizing the monitoring of patients after orthopedic dental treatment in order to prevent post-prosthetic complications.
