Дисплазия соединительной ткани (ДСТ) — системный фактор, влияющий на адаптационный потенциал организма, и предиктор ряда хронических заболеваний. Компоненты соединительной ткани широко представлены в структуре пародонтального комплекса и играют значительную роль в функционировании иммунной системы. В связи с этим, научный и практический интерес представляет изучение состояния локальной иммунологической активности и биомаркеров воспаления у пациентов с ДСТ и пародонтитом на фоне ДСТ. Цель. Оценка уровня провоспалительных цитокинов в ротовой жидкости у пациентов с недифференцированной ДСТ и пародонтитом. Материал и методы. Протокол исследования предполагал выявление и оценку фенотипических и висцеральных признаков ДСТ и оценку стоматологического статуса, на основании результатов которых исследуемые пациенты распределялись на 4 группы: группа сравнения — лица без признаков ДСТ и ВЗП (n = 15), I группа — пациенты без признаков ДСТ с ВЗП (n = 20), II группа — пациенты с ДСТ без ВЗП (n = 20), III группа — пациенты с ДСТ и ВЗП (n = 34). Проводилась оценка содержания ИЛ-1β, ИЛ-2, ИЛ-6, ИЛ-8, ФНО-α в ротовой жидкости. Результаты. У пациентов II группы определялась статистически значимо более высокая (относительно показателей группы сравнения) концентрация ИЛ-8, ФНО-α. Уровень ИЛ-2, ИЛ-8, ФНО-α при пародонтите и ДСТ достоверно выше аналогичных показателей в группе пациентов с пародонтитом без ДСТ. Заключение. Результаты, полученные в ходе настоящего исследования, выявили ряд особенностей спектра цитокинов РЖ у пациентов с ДСТ и специфику изменения данных показателей при пародонтите, протекающем на фоне ДСТ. В дальнейших исследованиях требуется уточнение прогностической роли выявленных статистически значимых изменений показателей, а также оценка и анализ их изменений при использовании различных современных методов лечения ВЗП у пациентов с ДСТ.

Relevance. Connective tissue dysplasia (CTD) is a systemic factor affecting the adaptive potential of the organism and a predictor of a number of chronic diseases. Connective tissue components are widely represented in the structure of the periodontal complex and play a significant role in the functioning of the immune system. In this regard, it is of scientific and practical interest to study the state of local immunological activity and biomarkers of inflammation in patients with CTD and periodontitis on the background of CTD. Aim of the study. To evaluate the level of pro-inflammatory cytokines in oral fluid in patients with undifferentiated CTD and periodontitis. Material and methods. The study protocol involved the identification and evaluation of phenotypic and visceral signs of CTD and assessment of dental status, based on the results of which, the studied patients were divided into 4 groups: comparison group – individuals without signs of CTD and IDP (n = 15), group I – patients without signs of CTD with IDP (n = 20), group II – patients with DST without IDP (n = 20), group III – patients with CTD and IDP (n = 34). The content of IL-1β, IL-2, IL-6, IL-8, TNF-α in oral fluid was assessed. Results. Group II patients had statistically significantly higher (relative to the comparison group) concentration of IL-8, TNF-α. The level of IL-2, IL-8, TNF-α in periodontitis and CTD was significantly higher than in the group of patients with periodontitis without CTD. Conclusion. The results obtained in the course of this study revealed a number of features of the spectrum of gastric cytokines in patients with DST, as well as the specificity of changes in these indicators in periodontitis occurring against the background of DST. Further studies require clarification of the prognostic role of the identified statistically significant changes in indicators, as well as assessment and analysis of their changes when using various modern methods of treating ICD in patients with CTD.
