Предмет. Агрессивные формы пародонтита ― группа заболеваний пародонта, для которой характерны быстрое развитие, значительная убыль костной ткани, невыраженные клинические проявления, слабые воспалительная реакция и иммунный ответ, а также небольшое количество микроорганизмов, не пропорциональное тяжести деструкции. Согласно современным представлениям об этиологии агрессивных форм пародонтита, основная причина их развития ― проникновение в ткани пародонта Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Prevotella intermedia и других пародонтопатогенных микроорганизмов. Поэтому агрессивные формы пародонтита могут требовать вспомогательной антибактериальной терапии. Цель ― определение и анализ наиболее эффективных методов антибиотикотерапии у больных с агрессивными формами пародонтита. Методология. Проведен обзор литературы ― публикаций отечественных и зарубежных исследователей, посвященных разработке подходов к повышению эффективности антибиотикотерапии агрессивных форм пародонтита у пациентов, с использованием научных поисковых библиотечных баз данных PubMed, Medline, Cochrane, Elibrary. Основной отбор материалов осуществлялся по ключевым словам с учетом критериев включения и исключения. Всего было найдено 37 публикаций. Результаты. В обзоре рассмотрены основные методики современной антибиотикотерапии при агрессивных формах пародонтита, отмечены основные преимущества и недостатки применяемых в настоящее время антибактериальных препаратов. Выводы. В современной пародонтологии наблюдается значительный интерес к проблеме антибиотикотерапии агрессивных форм пародонтита, изучаются различные режимы приема антибактериальных препаратов. Большинство исследований данной патологии посвящено использованию моксифлоксацина, азитромицина, а также комбинации амоксициллина и метронидазола. Дополнительное применение антибиотиков приводит к улучшению результатов лечения агрессивных форм пародонтита.

Background. Aggressive forms of periodontitis are a group of periodontal diseases characterized by rapid development, significant bone loss, few clinical manifestations, weak inflammatory and immune response, a small number of microorganisms, not proportional to the severity of destruction. According to modern ideas about the etiology of aggressive forms of periodontitis, the main reason for their development – the penetration of periodontal tissue Aggregatibacter actinomycetemcomitans. Therefore, aggressive forms of periodontitis may require adjunct antibacterial therapy. Objective ― to determine and analyze the most effective methods of antibiotic therapy in patients with aggressive forms of periodontitis. Methods. A literary review of the studies of Russian and foreign scientists on the development of approaches to improving the effectiveness of antibiotic therapy of aggressive forms of periodontitis in patients, using scientific search library databases PubMed, Medline, Cochrane, Elibrary. The main selection of materials was carried out by keywords. 37 publications were found. Results. The review describes the main methods of modern antibiotic therapy of aggressive forms of periodontitis, the main advantages of antibacterial drugs. Conclusion. In modern periodontics, there is a significant interest in the problem of antibiotic therapy of aggressive forms of periodontitis; various modes of antibiotics are studied. Most studies of this pathology are devoted to the use of moxifloxacin, azithromycin and a combination of amoxicillin and metronidazole. Additional use of antibiotics leads to better results in the treatment of aggressive forms of periodontitis.
