    <p>Актуальность. Прикрепленная десна — это часть комплекса тканей пародонта, плотно прикрепленная к надкостнице соединительнотканными волокнами. Она обеспечивает защиту зуба или имплантата от механической травмы любой этиологии. Ее отсутствие или недостаток негативно влияют на сохранность зубов и, особенно, имплантатов. В связи с широким развитием имплантологического лечения распространенность проблем в периимплантатной области очень высокая. Основой в лечении периимплантитов и их профилактике является создание оптимального объема (ширины) прикрепленной десны. В связи с большим числом различных персональных параметров и индивидуальных показателей, важных при оценке статуса пациента с периимплантной патологией в разных клинических случаях, в настоящее время не удается систематизировать методы лечения. Сегодня подход к выбору стратегии, тактики и конкретных методов лечения периимплантных патологий сугубо персонифицированный.&#13;
Цель исследования. Разработать алгоритм применения методов менеджмента тканей пародонта для хирургического лечения пациентов с периимплантной патологией.&#13;
Материалы и методы. В исследовании представлен и подробно разобран клинический случай аугментации прикрепленной десны в области имплантатов тоннельной методикой с использованием аутотрансплантата.&#13;
Результаты. Получен объем прикрепленной десны в области всех оперированных имплантатов, а также в области рядом стоящего зуба, адекватный требуемому объему периимплантных тканей, результат оценен по измеримым и визуализируемым показателям.&#13;
Заключение. Тоннельная методика с использованием свободного десневого деэпителизированного трансплантата позволяет создать необходимый объем прикрепленной десны в периимплантной области, сформировать высокое качество  мягких тканей десны; является методом первого выбора за счет малой инвазивности и полностью соответствует требованиям персонифицированного подхода.</p>

    <p>Relevance. The attached gum is a part of the periodontal tissue complex, attached tightly to the periosteum by connective tissue fibers. It protects the tooth or implant from mechanical injury of any etiology. Its absence or lack negatively affects the safety of teeth and, especially, implants. Due to the widespread development of implantological treatment, the prevalence of problems in the peri-implant area is very high. The basis for the treatment of peri-implantitis and its prevention is the creation of an optimal volume (width) of the attached gum. Due to the large number of different personal parameters and individual indicators that are important in assessing the status of a patient with peri-implant pathology in different clinical cases, treatment methods cannot be systematized at present. Today, the approach to the choice of strategy, tactics and specific methods of treatment of peri-implant pathologies is highly personalized.&#13;
The aim of the study is to develop an algorithm for the application of periodontal tissue management methods for the surgical treatment of patients with peri-implant pathology.&#13;
Materials and methods. The study presents and analyzes in detail a clinical case of augmentation of the attached gum in the area of implants by tunnel technique using an autograft.&#13;
Results. The volume of the attached gum in the area of all operated implants, as well as  in the area of adjacent teeth adequate to the required volume of peri-implant tissues, was obtained, the result was evaluated by measurable and visualized indicators.&#13;
Conclusion. The tunnel technique using a free gingival deepithelized autograft allows you to create the necessary volume of attached gums in the peri-implant area, to form high quality soft tissues of the gum; it is the method of first choice due to low invasiveness, and fully meets the requirements of a personalized approach.</p>
