Предмет ― дисфункция височно-нижнечелюстного сустава. Цель ― оценка клинической эффективности применения междисциплинарного подхода к проведению стоматологического ортопедического лечения пациентов с проявлениями синдрома обструктивного апноэ сна, нуждающихся в тотальной реставрации зубных рядов. Методология. Проведены комплексное обследование и лечение 95 пациентов (мужчины ― 52, женщины ― 43) с адентией, проявлениями синдрома обструктивного апноэ сна и патологией височно-нижнечелюстного сустава, нуждающихся в тотальной реставрации зубных рядов, которые были разделены на 3 группы. В группе 1 (32 человека) использовали стандартные методы ортопедической стоматологии, в группе 2 (29 человек) ― отдельные элементы междисциплинарного подхода к стоматологической ортопедической реабилитации, в группе 3 (34 человека) ― предложенный междисциплинарный подход к стоматологической ортопедической реабилитации. Наблюдение за пациентами, включенными в исследование, проводили в течение 7 лет. Ключевым этапом лечения пациентов группы 3 являлось полное последовательное обследование пациентов, комплекс мероприятий был дополнен изучением динамики проявлений синдрома обструктивного апноэ сна. Результаты. Установлено, что применение предложенного подхода к ортопедической реабилитации у пациентов основной группы способствует нормализации проявлений синдрома обструктивного апноэ сна: снижению индекса апноэ-гипопноэ и уровня минимальной сатурации, повышению показателей базальной сатурации эффективности сна и увеличению доли пациентов с благоприятными характеристиками сна. Выявленные изменения были стойкими, сохранение положительного эффекта отмечалось на протяжении 5―7 лет после начала лечения. Выводы. В рамках комплекса мероприятий по коррекции и восстановлению анатомической формы зубов одними из важных направлений междисциплинарного подхода должны быть оценка и купирование проявлений синдрома обструктивного апноэ сна и коррекция нарушений функции височно-нижнечелюстного сустава.

Subject ― is a temporomandibular joint dysfunction. Goal ― to evaluate clinical effectiveness of an interdisciplinary approach towards dental orthopedic treatment of patients with obstructive sleep apnea and a need for a full mouth reconstruction. Methodology. A total of 95 patients (52 males and 43 females) with adentia, obstructive sleep apnea syndrome and temporomandibular disorder were assigned to comprehensive examination and treatment. All patients were in need for a full mouth reconstruction. Patients were divided into 3 groups. Group 1 (n = 32) was exposed to standard treatment procedures. Group 2 (n = 29) received certain elements of an interdisciplinary approach to dental orthopedic rehabilitation. Group 3 (n = 34) was exposed to multidisciplinary approach towards dental orthopedic rehabilitation. The follow-up period was 7 years. A key step in the treatment of group 3 patients was to conduct full patient examinations sequentially. The corresponding procedures were supplemented with a study of OSA manifestation patterns. Results. It was found that the use of the present proposal contributes to the normalization of the OSA picture in patients with temporomandibular disorder. For instance, the apnea-hypopnea index dropped, the basal nocturnal saturation increased, and the minimum saturation level decreased. Overall, the proportion of patients with better to favorable sleep increased. The revealed changes were persistent, and a positive effect was observed throughout the 5―7 years of follow-up. Conclusion. Performing restoration of the anatomical shape of the tooth with the interdisciplinary approach, the dentist should assess and relieve the manifestations of OSA while correcting the temporomandibular joint disturbances.
