    <p>Исходные данные. Пожилое население – особая категория пациентов с высокой распространенностью стоматологических и соматических заболеваний. Лекарственная терапия сопутствующей патологии у пожилых пациентов повышает риски развития побочных эффектов и осложнений в стоматологической практике. Стресс и страх, связанные с болью и самой процедурой инъекции, могут провоцировать тахикардию, подъем артериального давления и привести к отказу от необходимого лечения.  Научные обзоры и статистика последних лет демонстрируют значительное снижение болезненности инъекции аппаратным введением местного анестетика в сравнении с традиционным. Однако, количество исследований, посвященных применению данной технологии у пациентов пожилого и старческого возраста с комплексной оценкой  гемодинамических параметров, уровня стресса, эффективности и безопасности анестезии, остается ограниченным.  &#13;
Цель – оценить эффективность и безопасность использования 4 % артикаинсодержащего местного анестетика с вазоконстриктором 1:200000 при аппаратном введении у пожилых пациентов при лечении стоматологических заболеваний в ретромолярной области.&#13;
Материалы и методы. Проведено проспективное сравнительное рандомизированное контролируемое исследование на базе стоматологической клиники ООО «Файвстарсдент» (г. Москва). Методом случайной выборки пациенты были распределены на две группы: первая группа использования карпульного шприца, вторая – использования компьютерного шприца «Injecta». Эффективность и безопасность анестезии оценивались по нежелательным реакциям и осложнениям после мандибулярной анестезии препаратом «Артикаин ИНИБСА».  &#13;
Результаты. Полученные результаты демонстрируют значительные преимущества аппаратного метода введения местного анестетика у пожилых пациентов по сравнению с традиционной техникой и согласуются с данными современной научной литературы.  &#13;
           Выводы. Аппаратный метод введения 4 % артикаинсодержащего местного анестетика с вазоконстриктором 1:200000 с использованием компьютерных систем доставки анестетика «Injecta» может быть рекомендован в качестве метода выбора для проведения местной анестезии у пациентов пожилого и старческого возраста при лечении стоматологических заболеваний.</p>

   <trans-abstract xml:lang="en">
    <p>Background.  The elderly population represents a distinct patient category with a high prevalence of dental and somatic diseases. Pharmacotherapy for comorbid conditions in elderly patients increases the risk of adverse effects and complications in dental practice. Stress and fear associated with pain and the injection procedure itself can provoke tachycardia, elevated blood pressure, and lead to refusal of necessary treatment. Recent scientific reviews and statistics demonstrate a significant reduction in injection pain with computer-controlled delivery of local anesthetic compared to the traditional technique. However, the number of studies on the application of this technology in elderly and senile patients, with comprehensive assessment of hemodynamic parameters, stress levels, and anesthesia efficacy and safety, remains limited.&#13;
            Objective.  To evaluate the efficacy and safety of a 4 % articaine-containing local anesthetic with a 1:200,000 vasoconstrictor administered via computer-controlled delivery for the treatment of dental diseases in the retromolar area in elderly patients.&#13;
            Materials and Methods.  A prospective comparative randomized controlled study was conducted at the dental clinic of Fivestarsdent LLC (Moscow). Patients were randomly allocated into two groups: the first group received anesthesia using a traditional cartridge syringe, the second group using a “Injecta” computer-controlled syringe. The efficacy and safety of anesthesia were assessed based on adverse reactions and complications following mandibular anesthesia with &quot;Articaine INIBSA.&quot;&#13;
            Results.  The obtained results demonstrate significant advantages of the computer-controlled method for local anesthetic delivery in elderly patients compared to the traditional technique and are consistent with contemporary scientific literature data.&#13;
            Conclusion. The computer-controlled delivery method of 4 % articaine local anesthetic 1:200000 using computer-assisted anesthetic delivery system “Injecta” can be recommended as the method of choice for local anesthesia in elderly and senile patients during dental treatment.</p>
