Актуальность. Тяжелые заболевания кишечника, к которым относятся болезнь Крона и язвенный колит, отличаются хроническим рецидивирующим течением, высокой частотой осложнений, неблагоприятным прогнозом, длительной лекарственной терапией и проявлениями в полости рта, такими как высокий уровень кариозного процесса, некариозные поражения второй группы (повышенная стираемость, клиновидные дефекты, эрозии эмали), гингивит, пародонтит, обильный налет на языке и травматизация слизистой оболочки. Эти особенности создают сложности при проведении качественной профессиональной гигиены, требуя максимальной атравматичности и точности от врача-стоматолога. Цель – подбор средств индивидуальной гигиены полости рта и разработка рекомендаций по использованию увеличительной техники при проведении профессиональной гигиены рта у пациента с хроническими заболеваниями кишечника (болезнь Крона). Методология. В представленном клиническом случае у пациента с болезнью Крона в стадии обострения был разработан и применен протокол профессиональной гигиены с комбинированным использованием трех оптических систем, что позволило эффективно снять поддесневые отложения без травматизации и болезненного растяжения чувствительной слизистой оболочки полости рта. Параллельно была скорректирована индивидуальная гигиена с использованием скребка для языка, увлажняющих и противовоспалительных средств. Результаты. После проведенного лечения наблюдается значительное улучшение показателей индексов гигиены (OHI-S = 0,5, WTC = 2), отсутствие кровоточивости десен (индекс SBI = 0), сухость слизистой оболочки, отсутствие воспаления в тканях пародонта. Выводы. Разработанные рекомендации по применению оптических систем являются ключевыми для проведения эффективной, щадящей профессиональной гигиены у пациентов с болезнью Крона и обеспечивают значительный визуальный контроль и минимальную травматизацию слизистой оболочки полости рта.

Relevance. Severe intestinal diseases, which include Crohn's disease and ulcerative colitis, are characterized by a chronic recurrent course, a high rate of complications, an unfavorable prognosis, prolonged drug therapy and manifestations in the oral cavity, such as a high level of carious process, non-carious lesions of the second group (increased abrasion, wedge-shaped defects, enamel erosions), gingivitis, periodontitis, plentiful plaque on the tongue and mucosal trauma. These features create difficulties in conducting high-quality professional hygiene, requiring maximum atraumatism and accuracy from the dentist. Objective. To select personal oral hygiene products and develop recommendations for using magnification techniques during professional oral hygiene procedures in patients with chronic intestinal diseases (Crohn’s disease). Methodology. In the presented clinical case, a patient with Crohn’s disease in an exacerbation stage was treated with a protocol for professional hygiene using a combined approach with three optical systems. This approach allowed effective removal of subgingival deposits without traumatizing or causing painful stretching of the sensitive oral mucosa. Simultaneously, individual hygiene was adjusted using a tongue scraper and moisturizing and anti-inflammatory agents. Results. Following the treatment, a significant improvement in hygiene indices was observed (OHI-S = 0.5, WTC = 2), with no bleeding on probing (SBI index = 0), no dry mucous membranes, or inflammation in the periodontal tissues. Conclusions. The developed recommendations for the use of optical systems are key for conducting effective, gentle professional hygiene procedures in patients with Crohn’s disease. They provide significant visual control and minimal trauma to the oral mucosa.
