Актуальность исследования высоты позиционирования ортодонтических замков (брекет-систем) обусловлена прежде всего возрастающими требованиями пациентов к эстетическим результатам лечения. Цель исследования — оценить влияние различных методов фиксации ортодонтических замков на линию улыбки. Материалы и методы. В исследовании приняло участие 56 человек в возрасте от 18 до 45 лет со смыканием зубных рядов по I классу Энгля, а также нормальной глубиной резцового перекрытия. Основным диагностическим методом являлся анализ фотографий зубных рядов и лица пациентов по М. Фрадеани, благодаря которому определяли уровень положения верхней губы, относительно верхних передних зубов. Результаты и обсуждение. При попарном сравнении значений экструзии и интрузии верхних передних зубов в группах 1 и 2 было выявлено преобладание значений экструзии при использовании метода TOM×TOM в сравнении с другими. Степень экструзии передних зубов в группах 1 и 2 методом MBT была меньше по сравнению с пациентами, у которых использовали способ TOM×TOM, но превалировала над показателями при методах Roth и Alexander. Для лиц с полным перекрытием коронок зубов и обследованных с визуализацией десны более 2 мм (группы 3 и 4) экструзия передних зубов отмечалась в большей степени при использовании методов TOM×TOM и MBT, но в меньшем объеме при методах Roth и Alexander. Кроме того, использование методов Roth и Alexander отличаются по преобладанию значений интрузии: бокового резца (метод Alexander), центрального и бокового резца (метод Roth) в сравнении с другими методами, в связи с чем у пациентов группы 3 и 4 рекомендовано применение указанных способов фиксации брекет-систем. Вывод. С целью создания привлекательной дуги улыбки необходимо придерживаться различных методов позиционирования брекет-систем в зависимости от исходного типа улыбки пациента. Полученные данные вносят весомый вклад в развитие ортодонтии, предлагая алгоритм фиксации брекет-системы у пациентов с разными типами улыбки.

Relevance of the studying of principles of orthodontic braces positioning is determined by increased demands of patients on aesthetic treatment results. The research is aimed at studying the influence of the height of orthodontic braces positioning on the aesthetics of smile line, which is a significant aspect of modern orthodontics. The aim of the study was to evaluate the effect of various methods of braces positioning on the smile line. Materials and methods. The study involved 56 people aged 18 to 45 years with class I occlusion and normal overjet. The main diagnostic method was the analysis of photographs of patients' dentition and faces according to M. Fradeani, which help to determine the upper lip position relative to the upper front teeth. Results and discussion. The pairwise comparison of the values of extrusion and intrusion of the upper front teeth in groups 1 and 2 revealed a predominance of extrusion using TOM×TOM method in comparison with others. The degree of extrusion of the anterior teeth in groups 1 and 2 by MBT method was lower compared to patients who used the TOM×TOM method, but prevailed over the indicators with the Roth and Alexander methods. For individuals with complete overlap of the dental crowns and those with gum visualization of more than 2 mm (groups 3 and 4), extrusion of the anterior teeth was noted to a greater extent with TOM×TOM and MBT methods, but to a lesser extent with Roth and Alexander methods. In addition, the use of Roth and Alexander methods differ in the predominance of intrusion values: lateral incisor (Alexander method), central and lateral incisor (Roth method) in comparison with other methods, and therefore the use of these methods of braces fixation is recommended in patients of groups 3 and 4. Conclusion. To create an attractive smile line, it is necessary to use various methods of braces positioning, depending on the initial type of the patient's smile. The data obtained make a contribution to the development of orthodontics, suggesting an algorithm for fixing braces in patients with different types of smiles.
